The the server instances above refer to the servers on which MaxScale is installed or does it refer to the number of database servers that each MaxScale instance monitors. For example can I have MaxScale installed on 5 application servers which then monitors two or less database servers without a license?

I hope you do not mind of topic response. In the near future I am looking to horizontal scale mysql/mariadb instances. What made you chose for maxscale and not something like ShardingSphere? I think I will mostly have an iops problem.
No problem. We were mainly looking at having automatic failover for our applications using our 3 node Galera Cluster and MaxScale seems to have been well suited for that as well as having other useful functionality.
I'm not quite sure how "server instances in production" should be interpreted; as the number of servers behind any single MaxScale instance or as the sum of all servers behind all your MaxScale instances. If the former, your example would be fine, if the latter it would not Perhaps MaxScale 21.06 (formerly known as 6.4) would work for you? That's GPL now, so there are no restrictions.
